Your First Dental Visit

  • Please arrive 5 mins earlier than your appointment time, so you could fill out the New Patient Form.

  • Your initial dental appointment would consist of an examination and review of treatment options. Usually, treatment would be started on the same day. However, additional evaluation and a second appointment might be needed if you have a complex medical history or treatment plan.

  • Please provide the following information at your appointment:

    1) Any previous x-rays that were taken by another dentist, if applicable. Please request their office to email your x-rays to us at dr.eileenwu@gmail.com. If the old x-rays are not in digital form, please collect the physical copies from the office and bring them to your appointment with us.

    2) A list of medications you are presently taking. Please let us know if you have a medical condition that may be of concern prior to the treatment or if you are currently taking any medications, such as heart medication, aspirin, anticoagulant, bisphosphonate, etc.

    3) If you have dental insurance, please bring the necessary documents and IDs. These will save time and allow us to help you process dental claims.

  • All patients under the age of 18 must be accompanied by a parent or guardian.

We accept most major PPO insurance plans, please see following for the example of PPO plans that we accept:

If your insurance is not listed, please give us a call to confirm acceptance. While we might be out of network with some insurance plans, fees may be similar or even less. We are happy to verify your insurance coverage prior to your dental visit to make your experience smooth and quick.

* Even though we don't accept Medicaid at this moment, we would provide affordable fees for your dental needs. Please give us a call for more information.


Payment

If you have deductible, co-insurance or need to pay out-of-pocket for any dental services, please use one of the following methods that we accept:

  • Cash

  • Checks

  • Money Order

  • Venmo

  • Zelle®
